The City of Farmington Hills is soliciting proposals from qualified firms to install durable, slip-resistant floor coatings in the apparatus bays and ancillary areas of five (5) Fire Department facilities. Location o Fire Station #1 - 35725 Nine Mile Road o Fire Station #2 - 28225 Middlebelt Road o Fire Station #3 - 29260 Grand River Road o Fire Station #4 - 28711 Drake Road o Fire Station #5 - 31455 Eleven Mile Road General Requirements - Bid Deposit Required - Insurance Required A. Access to the building will be made available to the Proposer for the purpose of their performing the work. Work shall be performed Monday through Friday, between 7:30 a.m. and 5:30 p.m., unless otherwise arranged with the Fire Department. B. The proposer shall perform cleanup of work area on a daily basis to remove debris from that day's work. At the completion of the work, the proposer shall remove all equipment, surplus material, rubbish and debris, etc., from the premises. All surfaces involved in the work shall be cleaned and made ready for use. Proposer shall remove all rubbish and debris from City property and legally dispose of it. C. All work shall be done in compliance with all applicable local, State and Federal laws, rules, regulations, zoning ordinances and building codes, as well as M.I.O.S.H.A. guidelines and Americans with Disabilities Act in effect at the time of the project. The proposer shall provide all barricades and other safety equipment, as necessary. D. The proposer shall provide experienced, capable personnel to direct and complete all services in a manner satisfactory to the City. All services shall be in strict accordance with all applicable codes, the highest quality prescribed by the applicable trade standards and by such specifications as are stipulated by the manufacturer. The proposer shall supervise all workmanship, including that of any subcontractors, to ensure that it is of the highest grade and of quality and according to the best standard practice. E. The proposer shall take all necessary measures to prevent damage to other areas of the building, grounds and utilities adjacent to his work. Should damage occur as a result of the proposer's work, the proposer is responsible for the repair and/or replacement of the damaged area. Otherwise, the City shall repair and/or replace the damaged area and charge the proposer or deduct the amount from the proposer's payment. F. Proposer should store his materials, supplies and equipment in a neat and orderly manner so as not to unduly interfere with the progress of his work, the work of other contractors or the operation of City business. Because the repairs will disturb essential operational areas of the building and essential emergency services are provided from this location, it is imperative that the work be scheduled in such a manner so as to be completed in an absolute minimum amount of time. G. Proposer shall not unload, move, hoist and/or store material or equipment, so as not to interfere with normal use of the facility and/or driveways, sidewalks or building entrances, except with prior written authorization by the City of Farmington Hills. H. The proposer is responsible for the storage and security of their equipment and materials and that of their subcontractors. Location of material and equipment storage, vehicle parking, and staging areas shall be established only after approval by the City of Farmington Hills Fire Chief or his authorized representative. I. A dumpster to discard cement dust, empty containers or other material or debris resulting from the project will be provided by the contractor at all locations. J. The City of Farmington Hills will provide 110v 20-amp electrical outlets as required. K. The project shall include but not be limited to: 1) Preparation o Removal of existing floor surface to achieve a texture to allow for a mechanical bond between the concrete substrate and Polyurea base coatings. o Diamond grind with dust collectors and the ability to isolate dust from living areas o Provide any repairs needed to existing floors, including patch all cracks and divots with quick cure epoxy patch or concrete replacement, once cured grind smooth. o Hand grind any areas that require special attention o Sweep and vacuum all areas to ensure a clean concrete surface o Clean entire area to receive floor system 2) Application o Apply basecoat of polyurea and Topcoat of polyaspartic and polyurea blend to the manufacture's specifications o Apply vehicle stripping along the driver's side o Finish will be anti-slip (Medium or Full Broadcast) TBD at mandatory walk-through o No tire lift is required Bonding Requirements - Bid Bond 5 % - Performance Bond 100 % - Maintenance Bond 25 % - Labor and Materials Bond 100 %
