Separate sealed Bids for replacing 6,000 linear feet of 2" galvanized waterline with 3" PVC pipe including a railroad crossing with 6" steel encasement Bid date desp- Each Bid must be submitted in a sealed envelope, addressed to Mayor Looney at 3713 Highway 367, Bald Knob, Arkansas 72010. Each sealed envelope containing a Bid must be plainly marked on the outside as Bid for Galvanized Water Line Replacement and the envelope should bear on the outside the Bidders name, address, and license number if applicable, and the name of the project for which the Bid is submitted. If forwarded by mail, the sealed envelope containing the Bid must be enclosed in another envelope addressed to the Owner at 3713 Highway 367, Bald Knob, Arkansas 72010. The Bidders attention is called to the requirements of the Supplemental General Conditions for the Revolving Loan Fund (RLF) contained in the contract documents including but not limited to Disadvantaged, Minority and Women's Business (DBE/MBE/WBE) participation and reporting requirements as well as, the "American Iron and Steel" requirement, Labor Standards, Equal Employment Opportunity, and the "Prohibition on certain telecommunication and video surveillance services or equipment". Further, Bidders will be required to submit with their Bids, a Contractor's Act of Assurance Form certifying their understanding of and compliance with the Supplemental General Conditions. In addition, post Bid completion of DBE/MBE/WBE Compliance Evaluation Forms will be required and approved before the Contract can be Awarded.
