Mill Hall Elementary - Concrete Work The Keystone Central School District is accepting proposals for the replacement of concrete sidewalks at Mill Hall Elementary School 210 Kyler Avenue Mill Hall PA, 17751. The Keystone Central School District proposes to enter into an agreement with a contractor to provide services to replace concrete sidewalks along Kyler Avenue and the Mill Hall Elementary School playground. The purpose of this proposal is to provide professional service to demo the existing sidewalks and then replace them with new. Service is to be provided under this proposal are as follows: 1) The Keystone Central School District proposes to enter into an agreement with a contractor to provide services to replace concrete sidewalks along Kyler Avenue and the Mill Hall Elementary School playground. 2) The purpose of this proposal is to provide professional service to demo the existing sidewalks and then replace them with new. 3) Contractor is to provide all necessary labor, material and disposal. 4) Contractor is required to dispose of all waste in accordance with all laws and regulations of the Commonwealth of PA. 5) Sidewalks to be minimum 6" thick 4000psi reinforced concrete. Approximately 260ft x 4ft. 6) Two ADA access cutouts with 2ft x 5ft warning pads will be placed at the Northwest corner of the playground to cross Kyler Avenue and to cross towards the Central Mountain Middle School. 7) Contractor shall carry insurance applying to all operations by them, their agents and employees, and shall provide proof of such coverage... 8) Contractor is responsible for protection of glass, other sidewalks, and pavement near the construction area. 9) Pennsylvania prevailing wages will apply. Prior to any contractor visiting the site for the purpose of gaining acquaintance with the site, the Contractor shall contact Supervisor of Property Services or designee by telephone (570-893-4900) EXT 2903 for notification and approval of visit. In the event that the product quality, delivery condition, or availability should become questionable by the "District", the "District" shall retain the greater of 5% of the bid award or 100% of the value of the item in question pending the resolution of the questionable item agreed upon by both parties.
