This project is an as-needed contract. Bid Bond or Certified Check required with bid: $2,500.00 Performance Bond required if awarded bid: Not Applicable The City of Danbury reserves the right to accept or reject any or all bids or any portion thereof, to waive technicalities, and to award the contract as deemed to best serve the public interest. Request for interpretation of any portion of the bid may be made by telephone to the Purchasing Agent at 203-797-4571. Pursuant to and in accordance with the Invitation to Bid, General Information for Bidders, and Specifications relating thereto, the undersigned hereby offers to furnish all labor, equipment, tools, supplies and other facilities and means necessary or proper to patch and pave utility trenches or road openings and to include all handwork and curbing as necessary as described in the specifications at the prices listed on the following pages. Scope of Work The City of Danbury is soliciting proposals for patching repairs to bituminous asphalt road ways. Typical patch repair will differ in size, ranging from small 5'x5' patches to full road width repairs of varying lengths. The contractor will be expected to provide a complete turnkey service, which may include one or multiples of the services items listed below, as required by the location. The work that is to be performed under these specifications is only to be directed by the Superintendent of Public Utilities, Assistant Superintendent of Public Utilities, or Chief of Water Distribution, Sewer Collection & Maintenance. Full payment for extended units is not guaranteed if direction by someone other than those listed above. Surplus material disposal costs are to be included in the Item unit price. The minimum charge listed in each Item represents the lump sum total cost to provide the Item service when minimum measures have not been reached. It is a requirement of this and all Danbury sub-contractors that OSHA standards are met and maintained for all applicable hazards associated with this project. Proof of OSHA training and compliance will be required of the low bidder in order to execute a contract with the City. Excavated patch locations cannot remain open overnight. The sub-contractor is not to open an area that cannot be closed by the end of the business day. Changes in weather conditions will not be a reason for incomplete and unsafe patches at days end. The only acceptable methods for closing a patch are tamped base and rolled asphalt. Under extreme circumstances, and only with the approval of the Superintendent of Public Utilities, will tamped base to final grade will be accepted. The latter must be excavated and paved the following day, no exceptions. It is the responsibility of the sub-contractor to comply with State Call Before You Dig regulations prior to commencement of excavation. All traffic control during road work must be supplied or performed by Proposer. Patches must be completed within 5 business days of notification from the City. Proposers must have access to hot patch throughout the entire calendar year. Cold patch asphalt will not be accepted. All proposers must OWN and have staff capable of operating the following equipment: tack truck, 5-ton vibratory roller, asphalt milling machine (4' minimum),
