The project shall be performed by the following Prime construction Contracts: HVAC Contract. The request must specify the name of the person requesting the bid package and provide a US mailing address, an email address, and a telephone number for such requestor. Bid packages will be emailed to the requestor, within two (2) business days of the receipt of the written request. No telephone requests for bid packages will be accepted, as all requests must be in writing. Bid security in the form of a certified check or money order payable to the Smithtown Special Library District or a satisfactory bid bond executed by the bilder in an amount equal to 5% (five percent) of the total amount of the bid shall be submitted with each bid. The surety issuing the bid bond shall have a minimum policyholder rating of "A-" as listed in AM Best's Insurance Guide and be licensed to conduct business in the State of New York. The contract will provide that every Laborer and workman employed on or about the work contemplated by the contract shall be paid not less than the rate of wages which are set forth in the prevailing wage rate schedule issued by the New York State Department of Labor. Each bid must be submitted on the official forms furnished in the bid package. All bids shall include a non-collusive bidding certification or statement required by Section 103-d of the General Municipal Law of the State of New York and shall also contain a non-segregated facilities certificate which shall provide that the bidder does not maintain or provide for his employees facilities which are segregated on the basis of race, creed, color or national origin, whether such facilities are segregated by directive or on a de facto basis. It is the Library Board's intention to award the contract to the lowest responsible bidder in compliance with the specifications, providing the required security, and who can meet the experience, technical and budget requirements. Each bid shall be firm for a period of ninety (90) days after the date set for submission of bids. Event Bid Description: Bids must be received at the Administration Office of the Smithtown special Library District located at 148 Smithtown Boulevard, Nesconset, New York 11767, at which time all bids will be opened and read publicly aloud in the Library's Community Room will the Main Level. Bids Proposals received after such date and time will be rejected.
