The work includes labor, materials, tools, equipment and incidentals as specified for the resurfacing of asphalt and/or concrete surfaces and other related work at various locations in Nassau County. The proposed roadways to be resurfaced may include, but are not limited to: Bates Road from NYC Line to approximately 350 ft east of Links Dr, Covert Avenue from Monroe St to LIRR south of North Ave, Feeks Lane from Bayville Rd to approximately 235 ft east of Factory Pond Rd, Harbor Hill Road from Roslyn Rd to Glen Cove Rd, Ludlam Avenue from Bayville Bridge to Bayville Ave, New Hyde Park Road from Jericho Turnpike (NY25) to Union Turnpike, Roslyn Road from Church St to approximately 300 ft south of Harbor Hill Rd, Searingtown Road from I.U. Willets Rd to LIE No. Service Rd, Searingtown Road from Harrow Ln to 300 ft north of Village Rd, Shelter Rock Road from LIE No. Service Rd to Northern Blvd (NY25A), South Avenue from Rockaway Ave to approximately 600 ft north of Rockaway Ave, Stewart Avenue from Covert Ave to New Hyde Park Rd, and Weir Lane from Forest Ave to Skunks Misery Rd. All bidders must submit a MWBE Utilization Plan with the Proposal and a completed Appendix C: Offeror's Affirmation of Understanding of and Agreement Pursuant to State Finance law 139-j(3) and 139-j(6)(b). Each bidder shall submit with its bid a separate sealed list naming each subcontractor to be used on the contract. All subcontractors must be approved in writing before beginning work. Only Contractors registered with the NYS Department of Labor may submit bids, and all subcontractors must hold the same registration. Security for Bid must be included with the bid submission. Bids received from unregistered Contractors or bids lacking required security or subcontractor information will be deemed non-responsive. Vendors must also comply with Nassau County Local Laws covering MWBE participation, service-disabled veteran contracting goals, and apprenticeship program participation as appropriate for the type and scope of the work to be performed.
