The project includes all labor, equipment, and materials necessary to improve the local sidewalks within the Utopia Subdivision in the Town of Cedar Lake. Work includes rehabilitation and reconstruction of existing concrete sidewalks and curb ramps, concrete sidewalk removal, curb and gutter removal, grading work, HMA surface patching, curb and gutter placement, placement of concrete sidewalks and curb ramps, installation of detectable warning elements, adjusting castings to grade, and maintaining traffic and homeowner access within the Utopia Subdivision. Layout and surveying for ADA compliance is required within the bid price. The work must be conducted in a timely, organized, and sequential manner, with sufficient resources to complete the work within the contract timeframe. Pre-qualification requirements: Contractors should be pre-qualified with the Indiana Public Works Certification Board prior to starting work on any local public works project. Bidders must be on the Bidder's Plan holder List in order to submit bids. Required submission documents include Bid security, Indiana State Board of Accounts Form 96, Bid forms, Affirmative Action Statement, Non-Collusion Affidavit, E-Verify Documentation, Iran Investments Certificates, Town Ordinance 1405, pre-qualification with the Indiana Public Works Certification Board, a preliminary project schedule, and execution/review of all applicable CDBG/Lake County project documents/requirements. A certified check, bank draft, or bid bond for 10% of the bid price must be included. Successful bidders must provide performance and payment bonds each for 100% of the bid price within fifteen days of the Notice of Award. Bids not meeting these requirements may be rejected. Contractors must also attend a mandatory pre-bid conference and submit questions in writing by the stated deadline.
