Sealed responses are requested for Structured Cabling projects. All bid responses must be submitted in duplicate in a sealed opaque envelope, labeled as specified, and include an electronic copy of all bid materials on a flash drive. No oral, telephonic, telegraphic, e-mail, or facsimile Bids will be considered. Bids will not be considered after the specified time. Bidders are responsible for timely delivery, and postmarks are not accepted as proof of receipt. A five percent Bid Bond of the total base Bid price must be submitted for Bids of $25,000 or more. Bidders must submit completed forms: Bid Signature Page (Appendix B), Bill of Materials (Appendix C), Familial Disclosure Affidavit (Appendix D), and a sworn and notarized Iran Linked Business Affidavit (Appendix E). The Bid must include a list of at least four references, one of which must be a school or school district, for similar work performed within the past three years. The submitted Bid shall include the full legal name of the Bidder and all details identifying the business entity. Materials, components, and equipment must be itemized individually with manufacturer, part number, unit cost, and quantity. No line items for miscellaneous materials or unspecified parts will be accepted. All Bidders must complete and submit an Intent to Bid Form. Walkthrough meetings at the facilities are encouraged for Bidders. Samples and/or brochures of proposed materials and equipment must be supplied upon request. Blueprints and drawings associated with the project will be provided upon request and at the Bidder’s expense.
