The contractor shall provide all labor, materials, and equipment necessary to repair roof leaks on Building 425 of the Louisiana Naval Air Station for the Department of Military Affairs as specified. A job site visit is required prior to bid opening, with a completed Jobsite Verification Form or signed letter from the agency representative as proof of visitation included in the bidder's submission. Bidders must have a Contractors' License Number in the appropriate classification(s) on the bid envelope and comply with all state and federal requirements. Instructions to bidders include: reading the entire bid and all attachments; submitting typed or inked bid prices or bidding online; manual signature in ink or online submission; including delivery of all items to specified destination; adhering to bid and performance bond requirements if applicable; returning all bids and addenda in a clearly marked envelope or online; complying with all applicable laws and specifications; certifying compliance with anti-collusion, non-discrimination, ADA, and related statutes; submitting bids on state-provided forms with appropriate authorizations.
