Please review the attached Terms and Conditions pertaining to the submission of this bid. The Affidavit for Filing with Competitive Bid and the Business Relationship Affidavit must have original signatures, be notarized and returned with your bid submission. Included with this packet: Invitation to Bid Terms and Conditions for Bidding Affidavit for Filing with Competitive Bid Business Relationship Affidavit W-9 to be completed by vendor Specifications/Bid Forms for Bid Item(s). This bid is submitted as a legal offer and any bid when accepted by the County constitutes a firm contract Purchases by Rogers County, Oklahoma are not subject to state or federal taxes. Firm prices will be F.O.B destination. Unit prices will be guaranteed correct by the bidder. Any additional products/pricing other than what is requested for bid will not be reviewed. Oklahoma laws require each bidder submitting a bid to a county for goods or services to furnish a Notarized sworn Affidavit for Filing with Competitive Bid and a Business Relationship Affidavit. (Affidavits are included in this bid packet.). The Board of County Commissioners will analyze all qualifying bid submissions. The lowest and/or best bid will be determined based on the following considerations: a. Price per unit of measure. b. Distance and delivery time to the desired destination. c. Product source (manufacturer, retailer, jobber) d. Product availability. e. Product quality. f. Vendor's reliability. g. Specific conditions identified by the vendors triggering a base increase. h. Any additional consideration as may be deemed appropriate by the Board of County Commissioners. Vendor does not need to return the entire bid packet with submission. The only documents required to be submitted are the Signed and Notarized Affidavits and the material item spread sheets with your noted pricing for the items being bid. Bidder agrees to defend and to save Rogers County from and against all demands, claims cost, expense, damage and judgements based infringement of any patent to goods specified in this order or the ordinary use or operation of such goods by the county or use or operation of such goods in advance with the bidder's direction Event bid description: When using a carrier such as UPS or Fed-Ex BOTH the interior and exterior envelope must be sealed and marked clearly with the word Bid, Bid Number and Closing Date. The Rogers County Board of Commissioners reserve the right to accept or reject any or all bids or award all or any portion of the items bid including awarding by districts on any items bid and to make the bid award in the best interest of Rogers County.
