The following is intended to represent some of the duties the selected Service Provider will provide for the City of Vallejo: Troubleshoot issues, perform repairs and install replacements of motorized garage/bay doors and electric gates, and other related type work, on an as-needed basis. Service Provider shall provide all labor, parts and supplies required to perform work in a complete and satisfactory manner. Service Provider shall be responsible for ordering and delivering to project sites any and all replacement parts/items. See Exhibit A of Appendix A. The City will work with the selected Service Providers on a case-by-case basis to determine the specific scope of services required. In order to be considered, all requested information shall be submitted. Please prepare and organize your proposal in a succinct manner based on the following requirements: 1. On the cover page of the proposal, please include: a. The title of the RFP “On-Call Garage Door & Electric Gate Repair-Maintenance”. b. Company name and the name, title and email address of the person who should receive the results of the selection. 2. Inclusion of promotional literature of a general nature will not be considered in the selection process. 3. Proposal content must consist of the following, and will be factored into the overall evaluation: A. Experience and Organizational Capacity (30%) B. Approach to Completing Scope of Services (30%) C.
