The work to be performed is OFFICE RENOVATIONS for the Housing Authority of the City of Batesville, Arkansas. All bids must be submitted on forms furnished by the Public Housing Agency (PHA), in duplicate, and in a sealed envelope marked to show the name and address of the bidder, contract job number, date, and time of bid opening. Bidders are required to have an Arkansas Contractors License when the cost of the work is $20,000.00 or more. The work is subject to the requirements of section 3 of the Housing and Urban Development Act of 1968, as amended. Each bid must contain: (1) signed bid form; (2) bid guarantee equal to five percent (5%) of bid price (certified check, cashiers check, U.S. Government bond endorsed to the PHA, or bid bond); (3) HUD-5369-A form; (4) Non-Collusive Affidavit if bid is over $50,000.00; (5) HUD-2530 form if bid is over $50,000.00; (6) Certification regarding Section 3 and Segregated Facilities; (7) Contractors Section 3 Plan Format; (8) Certification regarding Equal Employment Opportunity; (9) Certification regarding Non-Segregated Facilities; (10) Certification concerning Labor Standards and Prevailing Wage Requirements. The successful bidder will be required to furnish a Performance and Payment Bond in the amount of 100% of the full bid amount, in accordance with Arkansas law. The PHA reserves the right to reject any or all bids, accept or reject any line item, and to waive any informalities in the bidding.
