Addendum #1: Site Visit is scheduled for January 7, 2026. You will need to report to the visitor center by 1:00PM CST to be sponsored on base. Please email Betty Kliewer betty.kliewer.ctr@us.af.mil and Shannon Perry shannon.perry.1.ctr@us.af.mil if you are intending to attend and we will give you all additional required information. The scope of work includes mold and asbestos remediation at Vance AFB F713-112. Contractors are expected to address the removal and proper disposal of all mold and asbestos materials in accordance with applicable regulations. The work covered under this Statement of Work shall consist of the remediation of asbestos, mold and mildew along with any incidental work necessary to provide a complete and usable finished product. OCATION OF WORK A. Location of work to be accomplished under this statement of work is at Vance Air Force Base (AFB) Facility 713, Room 112. Vance AFB is located three miles south of the City of Enid, Oklahoma. Facility 713 is located east of the intersection of Goad Road and Kline Avenue. SCOPE OF WORK A. The Contractor shall furnish all supervision, labor, equipment, consumable materials, supplies, tools, and other items to complete the construction as listed in the Scope of Work. All quotes shall include all shipping charges for completion of the project. The contractor will coordinate all work with ASRC Federal Field Services (AFFS) Construction Management prior to execution. The list of items below are to be considered the general Scope of Work for the project. B. The Contractor shall field verify location of work, dimensions, and existing site conditions prior to commencing work. The Contractor shall contact AFFS Construction Management for any discrepancies with the Scope of Work. C. This project includes: abatement of asbestos, mold and mildew, and demolition of interior fixtures and finishes in room 112. Refer to APPENDIX C - RECORD AS-BUILTS and APPENDIX D - DRAWINGS for room layout and dimensions. Additional details are described below: 1. Owner to demolish the following items prior to start of Contractor work: a. Appliances - Microwave, stove, and refrigerator. b. Plumbing Fixtures - kitchen sink, bathroom toilet, bathroom sink. c. Electrical Fixtures - outlets, lights, light switches, etc. d. Fire Protection Fixtures - smoke detector(s), strobes, etc. e. Furniture - bed, night stands, desk, chairs, etc. f. Bathroom mirror. 2. PTAC air conditioning unit and exterior door to remain in place. 3. DEMOLITION: a. Contractor responsible for removing and discarding the following items (Refer to APPENDIX E - ASBESTOS REPORT for details on materials that tested positive for asbestos): 1. All drop-drown ceiling tiles and grid. 2. All interior doors, closet doors, and thresholds. 3. All drywall, including above the ceiling grid. 4. Wall insulation. 5. All ceramic wall tiles in bathroom. 6. All flooring, including carpet, Luxury Vinyl Tile (LVT), and ceramic tile within work area. 7. Floor transition strips. 8. All casework in kitchen and restroom, to include counter tops and back splash. 9. All thermal pipe insulation, pipe wrap, and white mastic around plumbing. Demo to nearest tee or five feet of piping, whichever is greater. 10. Bathtub/shower in bathroom. Includes basin, fixtures, and tiling. 4. Hazardous Materials a. ASBESTOS ABATEMENT: 1. Follow all recommended safety protocols prior to beginning demolition of pipe insulation, drywall, or blown-in insulation. 2. Dispose of asbestos waste in accordance with the Oklahoma Asbestos Control Act. Refer to the Oklahoma Asbestos Control Act for personal protective equipment (PPE) and certification requirements. Ensure all asbestos waste is disposed of at an accredited asbestos disposal facility. 3. Ensure abated asbestos wastes that are staged for disposal are sealed with minimum 6 mil poly sheeting or bags with asbestos hazard warning labels to prevent release of fibers during transport. 4. Ensure all, OSHA 29 CFR 1926, OSHA 1910.132-138 and NESHAP 40 CFR 61- Subpart M standards are enforced as they apply to the project. 5. Ensure abatement areas involving pipe insulation or "wet walls" are equipped with negative air pressure containment and door containment in the entry. Negative air containments will remain in place until such time as the required air clearance testing is completed, reflecting that the air is below the exposure limit for asbestos fibers. A copy is provided to the Civil Engineering Environmental (CEV) representative through AFFS Construction Management prior to project closeout. a. Cleaning - All abatement/sampling areas will undergo a thorough cleaning to ensure fibers do not remain in occupied areas and cleanup will be inspected daily by AFFS Construction Management. Detection of a dust layer in the abatement space will trigger recleaning until the space can be considered safe for occupation. b. Air Clearance Testing - Air clearance testing will be conducted by an Industrial Hygienist to determine if regulated asbestos fibers remain in the sampling/abatement areas. The Contractor will provide all air clearance testing results and an air quality report/certification to AFFS Construction Management within 10 calendar days of testing.
