Furnishing of labor, material, and equipment for the construction of a 1440 SF modular medical office with an attached covered porch. The building is to be set on a perimeter concrete masonry foundation with center piers. The work includes structural, mechanical, electrical, plumbing systems, civil work, utility extensions to the building, security fencing, concrete work, masonry, and other work as indicated in the contract documents. Bids are to be received for a single prime contract, and all proposals shall be lump sum. General contractors must have the appropriate license classification as required by the NC General Contractors Licensing Board. Each proposal must be accompanied by a cash deposit or certified check or bid bond equal to at least five percent (5%) of the proposal amount. A performance bond and a payment bond will be required for one hundred percent (100%) of the contract price. Payment will be based on ninety-five percent (95%) of monthly estimates with final payment upon completion and acceptance of work. No bid may be withdrawn for a period of 60 days after the scheduled closing time. The owner reserves the right to reject any or all bids and to waive informalities. Bidders must include the Identification of Minority Business Participation form and the appropriate affidavits with the bid proposal. Failure to complete these forms is grounds for rejection of the bid.
