Riverview Buckeye Lobby Remodel. All proposals must be submitted via email with the subject line Sealed Bid 25-28A Riverview Buckeye Lobby Remodel. Only emailed proposals will be accepted. Proposals must include: Contractor Information Form (TMHA Form No. 221), HUD 5369-A, Non-Collusive Affidavit (Form No. 212), Completed Original Bid Form and 10% Bid Bond, Statement of Qualifications with references on contractor letterhead, and HUD 2530. The successful bidder must complete all necessary forms to meet TMHA and HUD requirements and provide proof of Workers Compensation Insurance, General Liability Insurance ($1,000,000), and Automobile Insurance ($1,000,000). Bid specifications are available by emailing R.E. Warner. All questions regarding the drawings and specifications must be submitted by the deadline specified. A bid bond or certified check for 10% of the total bid is required, and a Performance Bond and Payment Bond each for 100% of the contract amount must be obtained prior to contract signing. Compliance with Davis-Bacon Regulations is required. TMHA reserves the right to review qualifications, experience, bid history, and past performance. All subcontractors must be approved in writing by TMHA and submit required forms. There is a goal to award 20% of contracts to minority business enterprises (MBEs). Firms debarred from federal contracts are ineligible. No member, officer, or employee of the local authority during tenure or for one year thereafter may have any interest in this contract or its proceeds.
