Hurricane City, located at 147 North 870 West, Hurricane, Utah will accept Bids and a Statement of Qualifications from qualified contractors to complete the Hurricane City 2026 Slurry Seal Project. Each BID and S.O.Q. must be submitted in a sealed envelope, addressed to Hurricane City, 147 North 870 West, Hurricane, Utah 84737. Each sealed envelope containing a BID and S.O.Q. must be plainly marked on the outside as BID for Hurricane City 2026 Slurry Seal Project and the envelope should bear on the outside the name of the BIDDER, his address, and his Utah Contractor's license number. If forwarded by mail, the sealed envelope containing the BID and S.O.Q. must be enclosed in another envelope addressed to the OWNER at Attn: Arthur LeBaron, 147 North 870 West, Hurricane, Utah 8473 Mobilization, Traffic Control, Slurry Seal, Type II Includes all costs associated with mobilizing, demobilizing, and Quality Control for the slurry seal project. Mobilization shall consist of preparatory work and operations, including, but not limited to, those necessary for the movement of personnel, equipment, supplies and incidentals to the project site, for the establishment of all offices, buildings, sanitary facilities and other facilities necessary for work on the project; and for all other work and operations which must be performed, or costs incurred, not otherwise paid for prior to beginning work on the various items on the project sites. This section includes preparation of construction scheduling, insurance, bonding and other incidental paper work required to initiate the project. Includes public relations, all of which shall be included in the lump sum price for the Mobilization item Includes all costs to furnish and maintain all signs and barricades needed to maintain safe and efficient traffic routes for the public. No work shall commence until the project has been adequately signed. All signs, barricades and channeling devices shall be kept clean and erected in accordance with the "Manual on Uniform Traffic Control Devices for Streets and Highways" (MUTCD). This item also includes all flagging operations to safely complete any and all bid items, or as directed by the Engineer; all equipment, personnel, protective devices, and traffic control as required to direct traffic away from the construction activities. Includes all traffic control devices, detour signing, as required by the Limitation of Operations. Includes all equipment, labor and materials required to clean and slurry seal surfaces identified on the project maps, or as otherwise directed by the Engineer according to the specifications and requirements included in the Contract Documents.
