Scope of work primarily includes but is not limited to preconstruction services, design documents, estimating, and BIM Coordination for prefabricated restrooms. Scope for Bid Package 1.4 - Hospital - Prefabricated Restrooms – Phase 1 includes special construction (prefabricated systems and products), plumbing (as applicable), heating, ventilating, and air-conditioning (HVAC) plant (as applicable), and electrical (as applicable). Instructions to bidders are to submit electronically a complete prequalification questionnaire and project data sheet in one file, PDF format. Prequalification Questionnaires and Project Data Sheet shall be emailed to Eva Lanie Cabreros, Contracts Administrator, with the subject title: 'SUBPQ 5551 HMCR – Prefab Restroom - '. All information requested in the forms must be provided in order to be considered responsive to the requirements of the prequalification. Every effort will be made to ensure equal access to contracts and other business opportunities. Each bidder may be required to show evidence of its equal employment opportunity policy. The successful bidder and its subcontractors will be required to follow the nondiscrimination requirements set forth in the bidding documents and to pay prevailing wage at the location of the work. The awarded contractor shall make best efforts to provide qualified SBEs and DVBEs with the maximum opportunity to participate and submit a small business outreach plan as part of prequalification. All contractors and subcontractors must be registered with the Department of Industrial Relations (DIR) at bid time. The successful bidder shall pay all persons providing construction services and/or labor on site no less than the UC Fair Wage ($15 per hour) and comply with all applicable working condition requirements. This project is subject to California Skilled and Trained Workforce requirements and compliance monitoring by the Department of Industrial Relations. Pre-qualification requirements: Each prospective contractor must submit electronically a complete prequalification questionnaire and project data sheet, in one PDF file. Subcontractors must be properly licensed by the State of California Contractors State License Board for the specified work (License Required: A or B or D-34). All contractors and subcontractors must be registered with the Department of Industrial Relations. Each bidder may be required to provide evidence of equal employment opportunity policy.
