Renovation of a medical facility in Louisville, Kentucky. Completed plans call for the renovation of a medical facility.
PROJECT DESCRIPTION: The project includes installation of a new PVC roofing system as an overlay over the existing SBS roofing on a sloped metal deck, replacement of select moisture-damaged insulation, all new metal flashing, and miscellaneous other minor upgrades to improve overall weathertightness, including treatment of rooftop mechanical penetration Bids are to be received from Prime Contractors on a Lump Sum Bid Basis for the entire project. All phases of work must be bid to and through Prime Contracting Firms. Bids must be submitted in the prescribed manner and on the official bid document form, following all specified conditions and requirements. Bids not meeting these requirements will be invalidated. The contract will be awarded to the lowest-cost responsive bid properly submitted by a responsible bidder. All qualifying requirements and forms are included in the Bid Document, and compliance with Kentucky workers’ compensation insurance requirements is mandatory. Preferences may apply to Kentucky resident bidders under established statutes and regulations. All Procurement Questions Shall Be Directed to Buyer of Record for This Project. THIS CERTIFICATION MUST BE COMPLETED AND EMAILED TO BUYER WITHIN 5 DAYS OF BID OPENING. Failure to submit this completed certification may be cause for rejection of the bidder’s proposal.
