The Grand View School District is seeking proposals for resurfacing the school cafeteria floor with a urethane cement-based flooring system, which includes surface preparation, installation, and final finishing. The intent is to complete the work with minimal disruption to school operations, thus vendors are encouraged to propose schedules that utilize after-school hours, weekends, or school breaks. The contractor will provide all supervision, supplies, labor, training, equipment, and materials needed for the project. Subcontractors may be used but coordination and payment remain the contractor’s responsibility. Specifications include: - Project area: cafeteria kitchen and adjacent high-traffic food service areas. - Surface preparation: mechanical grinding/shot blasting, crack repair, patching/leveling, cleaning/degreasing, and preparation for proper adhesion. - Urethane cement flooring system: commercial-grade system with primer, base coat(s), optional broadcast media, and a protective top coat (urethane or polyaspartic). - Slip resistance: aggregate-enhanced finish, cleanable and suitable for cafeteria use. - Transitions/edges: clean terminations at doorways and thresholds, 4” cove base optional. - Color/finish: minimum three standard color options with sample photos/chips. - Worksite safety: proper ventilation, barricades, signage, and safe access control. Vendor must list VOC levels and cure/return-to-service time. - Warranty: workmanship and manufacturer warranty, including maintenance requirements. Requirements: 1. Cove base height of 4 inches 2. Minimum product thickness of at least ¼” 3. Food service rated; USDA/FDA compliant 4. Seamless and non-porous 5. Resistant to thermal shock, moisture, grease, chemicals, and cleaning agents 6. Suitable for installation over existing concrete 7. Manufacturer and product name must be provided Evaluation criteria include price, project start date, completion timeline, references, and ability to provide the requested services. Instructions to bidders: Vendors must submit a sealed proposal and comply with all proposal conditions. Required documents include a signed and notarized non-collusion affidavit, signed pricing page, IRS W9, certification regarding debarment, declaration of sex offender affidavit, business relationships affidavit, three business references, bidder's bond (if required), certificate of insurance (general liability, workers comp, and auto liability), and a signed and notarized contract affidavit. Upon contract award, a performance bond, statutory bond, and defect bond will be required. If the project exceeds $50,000, the Competitive Bid Act of 1974 regulations will apply.
