Bidders must provide a full name and address with each bid and submit either a 100% bid bond or a certified check, cashier's check, or irrevocable letter of credit for 10% of the bid, made payable to the City of Bexley, Ohio. A 100% performance bond will be required for the faithful performance of the contract. Bidders must ensure non-discrimination in employment and submit several required documents with their bid, including: Proposal, Non-collusion Affidavit, Corporation Affidavit (if applicable), Affidavit of Personal Property Tax Liability, Bid Guaranty and Contract Bond, Certified Check Document (if applicable), Detailed Statement of Qualifications and EDGE Certification(s), Sub-Contractors List, Campaign Contribution Affidavit, and EEO Certificate of Compliance from State of Ohio.
