The project is for Garage Door Removal and Replacement at Fairway Manor. Contractors are required to refer to the detailed project general notes, specifications, and drawings provided in Exhibit 1 - Scope of Work. All work must be performed to the specifications noted in the exhibits. Exhibits are included as part of the solicitation and must be used to guide the work, though they do not need to be returned. The quote format should consist, at a minimum, of the following: - Bid Form - MBE/WBE/ACE Forms - Contractor Attestation Form - Bid Submittal Cover Sheet Proposals must include a completed MBE/WBE/ACE Participation Plan and ACE Worker Participation Plan, demonstrating the commitment to the specified goals for Minority-Owned (20%) and Women-Owned (10%) Business Enterprises, and ACE Program participation (25% ACE Worker hours, 5% Targeted ACE Worker hours). Only businesses listed in CMHA's directories are eligible for these designations. All contract documents, including any agreements from the contractor, are subject to review before execution. Services cannot be performed without a Notice to Proceed. Invoices must be certified and supported with necessary details. CMHA reserves the right to reject any and all bids, waive informalities, and seek new bids if necessary. The project is not subject to Davis-Bacon or HUD prevailing wage rates. Contractors must defend and indemnify CMHA and associated entities against all claims arising from the work. Required insurance includes Comprehensive General Liability, Automobile Liability, and Workers' Compensation Insurance. Fairway-Lorain, LLC and CMHA must be named as additional insureds on all relevant policies, and proof of insurance must be provided before starting work. All contractors are encouraged to thoroughly review the project scope in the exhibits and to attend the site visit to understand existing site conditions.
