The work consists of repairing the deteriorated interior faces of the existing parapet walls and penthouse walls and replacement of the roofing insulation and membrane including all parapet wall caps. Bidders must furnish all labor, materials, and equipment to complete the reconstruction in accordance with bid documents and specifications. All proposals must be executed on proposal forms as issued and bound with specifications. Proposals not conforming with this notice may be rejected. Each proposal must state the name and place of residence of the bidder and names of all persons interested. For corporations, only the President and Secretary names are required. A bid guaranty bond, a certified check, cashier's check, or letter of credit in the amount of 10% of the bid must accompany each proposal. The successful bidder must execute the contract and provide a 100% performance bond within ten days after the award. The address and telephone number of the bonding company and agent must be included with all bid guaranty and performance bonds. Bidders must comply with State Prevailing Wage Rates on Public Improvements in Butler County and the City of Middletown, Ohio. Contractors must certify compliance with the Ohio Bureau of Worker's Compensation's Drug Free Workplace Program to be eligible. Informal proposals or those containing unsolicited bids may be rejected, and the City reserves the right to waive informalities at its discretion. No bid may be withdrawn after deposit. The City reserves the right to reject any oral bids.
