Rockaway Township is accepting formal quotes for the ELPM ADA Ramps. The scope of work is detailed in Attachment B and the associated plan set titled "ADA Ramp Improvements - (10) Ramps Evelyn Place, Lenox Road, Patricia Lane, and Miller Avenue". All quotations must be submitted on the quotation page, Attachment A. Submitted quotes must include a copy of the vendor's Public Works Certificate. A New Jersey Business Certificate (BRC) and a W-9 form will be required. Indemnification and Insurance Certification and Endorsements must be provided and approved prior to commencement of the contract in accordance with Attachment C. This procurement is being conducted on a non-fair and open basis. The following items are required with your quote: Attachment A - signed quote sheet, Public Work Certificate, Business Registration Certificate, W-9 Form, Attachment C - signed, Attachment D - BED-C, C.271, Statement of Ownership Disclosure. A disclosure and certification of investment activities in Iran, Russia, and Belarus, as well as an insurance certificate, will be required after notification of job award and prior to commencement of the contract.
