Vault Pumping and Septic Service for Silver Lake State Park. Bidders must include their company name in the header of all documents submitted with their proposal. All documents and information submitted as part of a proposal will become public record immediately upon receipt by the State. Proposals may be posted on the State’s publicly available website after bidders are notified of the award recommendation. Bidders must follow the Proposal Instructions and provide a complete response to the required items. All required forms and schedules should be completed and submitted by the proposal deadline. Specifically, bidders must submit a Confidential Treatment Form, a Vendor Questions Worksheet, completed Schedule A – Statement of Work, and completed Schedule B – Pricing. All documents must be submitted electronically in a modifiable format (such as Microsoft Word, Excel, or Google Docs/Sheets). The price proposal should be saved separately from all other proposal documents. The proposal and attachments must be fully uploaded and submitted before the proposal deadline. Failure to meet these requirements may result in disqualification. Contact with any State personnel other than the designated Solicitation Manager regarding this RFP may result in disqualification.
