The project involves Residential LEAD Reduction, Mold Remediation, Electrical repair, Attic Insulation, and Gutter Services at 1035 Gentian Dr. S.E., Kentwood, MI 49508. The work includes addressing specific lead-based paint hazards as identified in the attached Lead Inspection and Risk Assessment (LIRA) Report, using abatement methods, interim controls, or both, as specified in the Work Write-Up. A lead clearance examination by a third party is required for impacted work areas after completion of lead-based activities. Only firms with active State of Michigan Lead Abatement Firm certification, employing at least one Lead Abatement Supervisor and the necessary number of Lead Abatement Workers, are eligible. Verification of active certification must be submitted with the response. For lead abatement projects, the required documents to remain on-site include the LIRA Report, Work Write-Up (Scope of Work) and any approved change orders, and an Occupant Protection Plan as approved by Kent County prior to activity.
