Location: Various streets within Lake Villa Township. Project length: 10,875 feet (2.06 miles). Proposed improvement includes HMA surface removal (2" and 4"), leveling binder (MM) N50, HMA binder course IL-19.0, and HMA surface course, Mix D, N50, together with appurtenant construction. Plans and proposal forms are available from Bleck Engineering Co., Inc. Prequalification is required; the two apparent low bidders as read must file an "Affidavit of Availability" (Form BC 57) within 24 hours after the letting. The Awarding Authority reserves the right to waive technicalities and reject any or all proposals. The following BLR forms must be returned by the bidder: Local Public Agency Formal Contract Proposal (BLR 12200), Schedule of Prices (BLR 12201), Proposal Bid Bond (BLR 12230) if applicable, Apprenticeship or Training Program Certification (BLR 12325), and Affidavit of Illinois Business Office (BLR 12326). Bid schedule quantities are approximate and for comparison purposes; payment will be for actual quantities of work performed and accepted or materials furnished. The scheduled quantities may be increased, decreased, or omitted as provided. Submission of a bid warrants that the bidder has examined the plans and understands all requirements. The bidder is responsible for proposal errors from failure to conduct an in-depth examination. Any special envelope provided should be used to submit the proposal. When mailed, the sealed proposal should be addressed to the Awarding Authority at the specified address. All proposals must be filed before the specified time and place. Late proposals will be returned unopened. Bidders may withdraw proposals before the opening time, by written request or in person.
