Sealed bids are invited for the following items: Superpave Surface Course 9.5, Superpave Base Course 19, HMA For Patching, Hot Mix Asphalt Curb, Hot Mix Asphalt Berm Type A, Reclaimed Pavement For Base Course, Cold Planing and Profiling of Bituminous Surfaces, Structures Casting Adjustments, Manhole and Catch Basin Frames & Covers and Gate Boxes, Sand, Stone and Gravel, On Call Side Walk Repairs, Bituminous Concrete Type I-1, and Bituminous Concrete Cold Patch (F.O.B. and Delivered). Certain items require MassDOT prequalification in specified classes of work with estimated project values provided for each. The bid forms must be filled out and signed as directed, sealed in an envelope with the name and address of the bidder, the project title, and the Item Number. Each bid must be accompanied by a certified or cashier's check or bid bond in the amount of 5% of the total bid price, payable to "City of Methuen". Deposits will be returned within ten days after the bid opening to all but the three lowest bidders; for those, upon execution of the contract. If the successful bidder fails to execute the contract, the bid deposit will become the property of the City. No proposal may be withdrawn within thirty days after the bid opening, excluding Saturdays, Sundays, and legal holidays. Prospective bidders for specified items must be prequalified per 720 CMR 5.00. Only bidders listed in the official or waiver contractor lists issued by the MassDOT Prequalification Office may obtain an official proposal sheet. Bidders must certify their ability to furnish labor in harmony with all elements of labor and that all employees have completed at least ten hours of OSHA approved training. The successful bidder for certain items must pay workers at minimum wage rates established by the Commissioner of Labor and Industries. The Awarding Authority reserves the right to reject any or all bids and to waive any informalities in the bidding if deemed in the city's interest.
