Abatement and replacement of a roof at 3 Johns Hollow Road, Old Field, NY, for People with Developmental Disabilities, NYS Office for Long Island DDSO. Attendance at a mandatory site visit is required prior to submission. Quotes must include a detailed cost breakdown (materials, labor, and overhead) and use the provided quote proposal form. Prevailing wages must be paid as this is a NYS Public Works Project. Vendors and subcontractors must be registered with NYS DOL and submit a copy of the certificate with their quote. Contractor is responsible to verify all measurements and quantities. 2. Construction drawing, sketches, and or plans maybe supplied and are to be followed along with the scope. 3. Abatement work of flat roof (approx.1250 sqft) to be completed first. Contractor is responsible to coordinate with air monitoring vendor (Damar Environmental Corp). 4. Remove and dispose all roofing (Approx. 4000 sqft+1250sqft=5250sqft), and all related materials and accessories. 5. Replace any rotted roof deck sheathing with new 5/8" thick exterior grade plywood sheathing (approx.10 sheets). 6. Furnish and install ice and water adhesive membrane shield (or equal) over entire roof deck sheathing. 7. Furnish and install new drip edge. 8. Furnish and install Timberline UHDZ asphalt shingles (or equivalent) and new ridge venting on pitched roofing. 9. Install new fascia and soffits, repair supports were needed. Furnish and install new vented soffit and cap all fascia with white wood grain textured aluminum. 10. Repair any rotted wood and cap all trim around doors and windows. 11. All roofing materials, accessories, installation and fastening shall be installed following manufacturers' requirements. 12. Furnish and install new aluminum flashing and caulking around chimneys, all vent pipes, all penetrations or where required for prevention of water infiltration. 13. Furnish and install new five (5) inch aluminum seamless roof gutters, leaders and downspouts. Furnish and install new Amerimax metal mesh leaf gutter guards (or equivalent), gutters shall be installed properly pitched to new downspouts. 14. Contractor will supply any third-party inspections and certifications where required. 15. All work to be done, to NYS building codes and regulations. 16. OPWDD is the AHJ for the site. Prior approval is required for all substitutions.
