Renovation of a mixed-use development in Reno, Nevada. Completed plans call for the renovation of a 2,016-square-foot medical facility.
1. This project will be title "Replace Dietetics and Cath Lab Flooring". The project number is 654-22-003. 2. Project scope is replacement of flooring in the Cath Lab & Dietetics Services (kitchen) spaces located in Building 1 and 1D. The contractor should replace approximately 850 sq. ft of the Cath Lab floor, and 1,200 sq. ft of the kitchen space as shown on the Drawing labeled "Cover Sheet" GI-000. The work will include demolition, recycling and disposal of existing flooring, and installation (new construction) of new flooring. The work should also account for temporary removal, storage, and reinstallation of furnishing and equipment as required for the successful replacement of flooring products as well as infection control and life safety requirements. 3. The contractor must completely prepare site for building operations, including demolition and removal of existing flooring, and furnish all services, materials, supplies, temporary equipment and items, tools, specialty services, and project supervision. 4. Contractor will perform work for the Reno VA as required in this Statement of Work, Drawings, and Specifications. All work will conform to the facility requirements as outlined in the 01 00 00 General Requirements Specification. Construction must follow all applicable VA, Federal, State, County, City, Industry design guides, codes, standards, regulations, and policies. 5. Should any questions or concerns arise during construction, a Request for Information (RFI) will be submitted to the Contracting Officer (CO) and Contracting Officer Representative (COR) for further clarification, prior to proceeding with work. 6. All work is located at the VA Sierra Nevada Health Care System facility located at 975 Kirman Ave., Reno, Nevada 89502. SCOPE 1. The general scope of work is to include all material, supplies, equipment, labor, oversight, and administration to install a functioning complete system. See attached drawings and specifications for specific requirements by trade: a. Building 1D, Cath Lab: 1. Temporary removal of equipment and furnishings as required for the successful replacement of flooring products. 2. Demolition, recycling, and disposal of existing underlayment and welded sheet vinyl flooring. 3. Subsurface preparation. 4. Installation (new construction) of new welded sheet vinyl flooring. b. Building 1, Dietetics 1. Temporary removal of equipment and furnishings as required for the successful replacement of flooring products. 2. Demolition, recycling, and disposal of existing carpet flooring. 3. Subsurface preparation. 4. Installation (new construction) of new welded LVT flooring. 2. Contractor to phase work and coordinate with facility to minimize disruptions to normal operations. Contractor will work with the COR to determine impacts and timelines. 3. Work is to be performed during normal work hours Monday - Friday between the hours of 7:30am and 4pm, excluding Federal holidays. After-hours work may be required during work phases that generate high levels of noise/vibrations or produce noxious fumes that cannot be contained to the work area. The proposed work will be performed to cause minimal disruption to VA staff and patients. Any after-hours work will be agreed upon with the Contracting Officer's Representative (COR) prior to the work commencing. 4. Work area must be cleaned prior to leaving the site. Any hazards must be identified, and risk properly mitigated to allow normal access through t e corridors during regular hours. 5. No utility shutdowns will be allowed without proper prior coordination with the medical center. Minor utility shutdowns (those which in no way impact patient care activities) are to be scheduled no less than 72 hours in advance of the planned shutdown. Major utility shutdowns (those which do impact patient care activities) are to be formally requested no less than 21 days in advance of the requested shutdown. 6. COR Notification: No contractor is permitted to perform on-site contract work without COR knowledge. 7. There is limited space available for the contractor to use as a staging location. Unless otherwise noted in the contract drawings or specifications, all staging of equipment and materials is to occur within the boundaries of the limits of construction shown on the contract documents. Coordination for street use for dumpsters, storage containers, or office trailers is between the contractor and the City of Reno.
