Furnish various types of aggregates as listed: approximately 2,000 tons of M.W.S. patch mix (F.O.B. plant), approximately 2,000 tons INDOT 402 type B surface intermediate and base (F.O.B. plant), approximately 15,000 tons coarse aggregate (F.O.B. plant), approximately 5,000 tons coarse aggregate (delivered), approximately 15,000 tons fine aggregate (F.O.B. plant), approximately 5,000 tons fine aggregate (delivered), approximately 1,000 tons sand (F.O.B. plant), and approximately 1,000 tons sand (delivered). The awarded contract may be extended for one-year periods by mutual agreement. Unless otherwise specified, each item shall be governed by Indiana Department of Transportation (INDOT) Standard Specifications. All bids must be submitted electronically through Ion Wave Technologies and must include a Non-Collusion Affidavit, a bid bond or certified check for ten percent (10%) of the bid amount, and a notarized Comprehensive Compliance Form (covering Affirmative Action, government of Iran disqualification, drug free workplace, OSHA/IOSHA, and employment eligibility). Failure to provide all required documents may result in a material defect in the bid. The Board of Public Works and Safety reserves the right to reject any or all bids.
