Demolition of identified buildings and slabs, including all building materials, building contents, and other identified materials at 14669 State Highway 27, Onamia, MN 56359. Fill to grade area of removal/demolition and finish with topsoil. Proper well abandonment/closures with required paperwork and proper septic abandonment/closure with required paperwork. Contractor is responsible for securing all required permits and notifications, paying all related fees, and providing copies of permits and notifications to the Mille Lacs Band of Ojibwe Department of Natural Resources, and other relevant authorities if needed. Upon completion, the contractor must provide copies of abandonment and closure forms, and receipts/logs from qualified landfills for all materials removed. All proposals must include: itemized cost for building demolitions, filling of areas, and permit/notification costs; itemized cost for wells and septic closures; estimate of days required to fulfill the scope of work; detailed plan of site safety and security during work. Bidders must provide a consulting firm's proposal, a copy of current Mille Lacs Band Vendor's License or application, certificate of insurance for commercial liability ($1 million) and design professional liability ($2 million), W-9, list of subcontractors (if any), and any other project-specific information requested. Proposals must be mailed and labeled as specified; hand deliveries or use of carriers that do not deliver to P.O. boxes will not be accepted and proposals not submitted in this manner will be rejected.
