Furnish and place 53 tons of flexible base replacement, 258 tons of asphalt leveling wearing course, 366 tons of asphalt base course leveling, 194 tons of asphalt wearing course, 18 tons of paved drives, 400 linear feet of double yellow paint, and one survey to maintain positive drainage. All work and material must meet current Pennsylvania Department of Transportation Specifications Form 408 and its supplements. A bituminous escalator clause is included in this bid. Performance bond and payment bond in the amount of 100% of the contract, as well as proof of worker's compensation insurance, will be required from the lowest responsible bidder. Proposals must be upon the forms furnished by the municipality. The municipality reserves the right to reject any or all proposals. DESCRIPTION OF WORK: Flexible Base Replacement Contractor to saw cut or mill existing roadway in failed areas. Saw cutting, excavation, hauling and disposal, bituminous tack coat, bituminous material, and sealing of the joints are considered as incidental. Contractor to follow Pub 408 Section 316. Asphalt Wearing Course 9.5 MM (Fine Grade) PG 64S-22, <0.3 Million ESALS, SRL-L (Leveling)(2" Depth) Contractor to furnish and supply, place and compact wearing course as tabulated. Contractor to follow Pub 408 Section 410. Tack Coat is incidental to paving item. Asphalt Base Course Leveling 25 MM, PG 64S-22, <0.3 Million ESALS, SRL-L Contractor to furnish and supply, place and compact base course. Contractor to follow Pub 408 Section 313. This work may require handwork as well as layout, survey, and grade verification to fill in dips and depressed area. Asphalt Wearing Course 9.5 MM, (Fine Grade) PG 64S-22, <0.3 Million ESALS, SRL-L 1/2 " Scratch, 1 1/2 " Wearing Contractor to furnish and supply, place and compact scratch course prior to placing Wearing Course. Tack coat is required prior scratch and wearing course placement and is incidental to this item of work. Contractor to follow Pub 408 Section 410. Paved Drive (Includes Milled Paving Notch Tack Coat and Joint Sealing) Contractor to pave driveways. This work includes a milled paving notch at tie-in, tack coat, Wearing Course 9.5 MM Fine Grade, compaction, and joint sealing. Contractor to follow Pub 408 Section 410. Double Yellow Line Paint This is the application of 2 yellow centerlines measured as linear foot for both, as directed by municipality. Contractor to follow current Pub 408 Section 901 and 962. Centerline layout is included with this item of work. Survey To Maintain Positive Drainage Contractor to design and consult with Penn Township for profile and grade requirements for tie-ins. Contractor to supply survey crew for layout. The municipality will withhold payments if all the above requirements are not met. Note: Incidental to the above items is mobilization, cleaning of roadway, and traffic control according to Pub. 213. Manhole and water valve boxes are incidental to paving. Contact Township to coordinate size and quantity. Notify Township 5 Days before paving operations begin.
