Description of the Project: - Replacing HVAC in the kitchen of the nursing home. Bidder Requirements: 1. Each bidder will provide a performance bond if the project is estimated to exceed $25,000. 2. Compliance with prevailing wage requirements. 3. Laborers to receive mandatory safety training. 4. Laborers to be Missouri residents or residents of approved states when excessive unemployment restrictions apply. 5. Contractors bidding on a contract for services in excess of $5,000 to provide an affidavit and supporting documentation that affirms the contractor's participation in a federal work authorization program, such as E-Verify, and that the bidder will not employ illegal workers for the project. A contractor is only required to provide this affidavit to the district annually. 6. Proposals shall be accompanied by a Certificate of Insurance and provide for a minimum of $1,000,000 General Liability coverage $2,000,000 Aggregate coverage, and Workman Compensation insurance. 7. Current Prevailing wage order will be followed. 8. All materials and labor are guaranteed to be as specified. 9. All work is to be completed in a workmanlike manner according to standard practices. 10. The total dollar amount will be due at completion of the job. Scope of Work: A. New 5-ton Air Conditioner with Indoor Air Handler B. Deliver and install a new 5 ton 208/230v 1ph Air Conditioner C. Place the new outdoor unit outside of the building on the roof where the existing unit is set. Install new electrical wiring whip. Install surge protector. Install low ambient cooling kit. This allows the unit to operate in cold weather. Install crankcase heater. D. Install new Air handler. Suspended from the ceiling. Hook to existing supply air ductwork. Hook up to existing low voltage wiring. Hook up to existing high voltage wiring. E. Flush existing copper line set. F. Install new digital thermostat. G. Run 2/4 PVC Drain line to the nearest floor drain. H. Warranty: 1-year labor, service calls on regular time: 10 - years all parts, 10 years Compressor & 5 years on the thermostat. This warranty DOES NOT include changing filter, parts damaged by high voltage, lightning damage, varmints, flooding, or repairs made by other services. Instructions to Bidders: Bids must be submitted to the county clerk in a sealed envelope marked "Daviess County Sidewalks". Bidders are also asked to state the company's start date for this request. Bids will be due at the Daviess County Clerk's Office. Bids will be awarded only when approved by the Daviess Commissioners. The Commissioners have the right to choose the most appropriate bid at that time, which might not be the lowest bid. Projects may begin after Commission approval. Bidder is to provide all bonds, insurance, materials, tools, equipment, and supervision.
