OPWDD is seeking quotes from responsible and qualified MWBE, SDVOB, or New York State small business contractors to provide sidewalk and patio replacement service at Sunmount DDSO, SOIRA 9 Depot Street. This opportunity is limited to New York State Small Businesses, businesses certified under Articles 15-A and 17-B of the New York State Executive Law (Empire State Development Corporation certified Minority or Woman Owned Businesses), or New York State Office of General Services Service-Disabled Veteran-Owned Businesses. Contractors must provide certification documents when submitting a quote. Work is to be completed within 30 days of award Patio and sidewalk replacement. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project, and prevailing wages are to be paid, and Certified Payrolls must be submitted at time of completion. Refer to attached specification for the following scope of work. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of the patio and sidewalk replacement, before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent or his/her representative. SCOPE OF WORK includes, but is not necessarily limited to the following: 1. Removal of existing sidewalks and patio and proper disposal of materials. 2. Excavate as necessary to proper depth required for new patio and sidewalk installation. New patio and sidewalks shall retain the same dimensions as existing. 3. Prepare subgrade, infilling where necessary. 4. Compact subgrade as necessary to accept subbase and concrete. 5. Provide and install subbase material. 6" minimum depth is required. 6. Provide and install appropriate bedding material. 2" minimum depth is required 7. Install forms as required for new patio and sidewalk. 8. Pour approximately 1350 Square feet of new concrete. 4-foot-wide sidewalk slabs and approximately 400 square foot patio area. Concrete shall be a minimum of 5 inches thick throughout the project area. Patio Thickened edge slab shall be 6 inches thick at a width of 12 inches around the perimeter of the patio area. Exact dimensions to be calculated by contractors during site visit. New prep work and concrete to meet OPWDD design standards for walkways. 9. New concrete shall have a 4000-psi compressive strength or better. 10. Finished concrete shall have a broom finish. 11. Control joints are to be cut where necessary to prevent future cracking and deterioration. 12. All preparation and finish work shall comply with standard practices and applicable codes.
