Scope of Work: Reclaim approximately 0.43 miles of Heater Road together with roadway repaving, driveway apron repaving, utility adjustments, common excavation, pavement re-striping and shoulder gravel work. Excess reclaim may be used for shoulder gravel. Instructions to Bidders: All bids must be submitted through the City of Lebanon eProcurement Portal. The Bid Proposal should give a unit price in figures for each item yielding a total price for each item, entered by a company representative authorized to commit the company. Contracts will only be awarded to responsible contractors who have adequate financial resources, necessary experience, organization, technical and professional qualifications, skills, equipment, and facilities, can comply with the required schedule, and have a satisfactory record of performance and completion of similar projects. Bidders must examine all contract documents, visit the work location, and inform themselves of the work prior to proposal submission. All bids must be accompanied by a Bid Bond in the amount of 5% of the Bid Price. The successful bidder must execute a Performance Bond and a Payment Bond, each in the amount of 100% of the Bid Price, within 5 business days of the Notice of Award. Bid quantities are subject to change based on actual field measurements taken during construction. The City reserves the right to accept or reject any or all bids and to cancel the request in part or in its entirety. The proposal must include all information requested in the vendor questionnaire and the bidder must confirm completion of all work as described in the contract documents per the lump sum and/or unit prices entered in the pricing list.
