Work under this contract consists of installation of approximately 1,103 feet of 12-inch Ductile Iron water main and appurtenances by open cut, 150 feet of 12-inch Ductile Iron water main via Jack and Bore, installation of 16-inch gate valve on existing water main, water service connections, storm sewer improvements, water main abandonment, and related restoration work. All bids must be submitted in a sealed envelope and shall contain, at minimum, the Bidder's Qualification and Experience Statement (QES-1-4), Proposal (P1-4), Designation of Major Subcontractors and Suppliers (MS-1), Legal Status of Bidder (LS-1), and Bid Certification (BC-1). Each Proposal must include a cashier's check, certified check, money order or bid bond, payable to the owner, in an amount not less than five percent (5%) of the amount of the Proposal, as Bid Security. The successful Bidder will be required to furnish a satisfactory Performance Bond and Labor and Material Bond, each for one hundred percent (100%) of the Contract. Withdrawal of any Proposal is prohibited for 90 days after the opening. Bidders must purchase a set of Plans and Specifications and be a registered Plan Holder to submit a bid.
