Question Deadline: 4:00 P.M. on May 22, 2026 All questions concerning the Project should be in writing via email and directed to Sharon Fallon (Sharon.Fallon@cpl.org) Lakeshore Facility Parking Lot Resurfacing Cleveland Public Library 17001 - 17133 Lakeshore Boulevard Cleveland, OH 44110 The project consists of removing and replacing the top two and one quarter inches (2.25") of asphalt at the Cleveland Public Library's Lakeshore Facility, which is located at 17001-17133 Lakeshore Boulevard in Cleveland, Ohio. The parking lot is approximately 43,200 square feet in area, and the selected contractor should mill and recycle the existing asphalt to the extent possible. Additional supporting tasks are also to be a part of this project and are described in the Contract Documents. The Contractor is responsible for scheduling the project, coordinating the subcontractors, and providing other services identified in the Contract Documents. Prevailing Wage Rates and Equal Employment Opportunity requirements are applicable. Subcontract agreements must comply with the Subcontract Form # C190-00 52 14 for Limited Scope Projects, as found on the Ohio Facilities Construction Commission website. Bidders are encouraged to be enrolled in and to be in good standing in a Drug-Free Safety Program (DFSP) approved by the Ohio Bureau of Workers' Compensation prior to submitting a bid, and if awarded a contract, shall comply with other DFSP criteria described in Section 1.3 of the General Conditions. Bids must be sealed. Per the owner, as part of Addendum #01, there was information specific to another project (The Main Fire Alarm Project) accidentally included in the addendum.
