Please submit a quote on Hot Mix Paving to be furnished and delivered, with paving, clean-up of the streets, and traffic control using your employees and equipment. Base your price for PG64-22 asphalt cement at $708.08 per liquid ton as established by TDOT’s Bituminous Index for the month of May 2027. Any fluctuation of the asphalt cement up or down greater than five percent will result in a price increase or decrease from the base bid, as determined by the Tennessee Department of Transportation in Special Provision 109B. All driveways and entrances/exits will be ramped with asphalt ramps as far back as needed to provide proper drainage and proper roadway entrances. All work is to be performed and constructed according to TDOT specifications, latest edition. The winning bidder will be awarded the contract on both asphalt products. Any fuel surcharges must be submitted in advance to City Council for prior approval, which may result in a rebidding of the Price Contract. Instructions to bidders: Each Request for Quote should be in a separate envelope and have typed/noted on the envelope the CRO#, item, opening date, and time. Mailing envelope should be addressed to City of Crossville, ATTN CITY CLERK/BIDS. No electronic submissions will be accepted. Specifications are intended to be open and non-restrictive. Reference to brand names, catalogs, etc., is to establish minimum standards of quality. All bidders must state brand name and catalog number of product proposed. All prices quoted should be on a delivered prepaid basis to the F.O.B. destination shown in the shipping instructions. Insert time discount terms, if any, in the space provided. The City of Crossville is exempt from sales tax with respect to materials it purchases for municipal projects; however, the contractor is liable for use tax on those materials. Unless otherwise indicated, quotations should be submitted on this form indicating unit price, total extension of each item, and grand total. In case of error in extension prices, the unit price will govern.
