Site work and paving for a sidewalk / parking lot in Yorktown Heights, New York. Completed plans call for site work for a sidewalk / parking lot.

Gomer Driveway Resurfacing: There is a mandatory scheduled site visit. When submitting a quote, include a cost breakdown (materials, labor, and overhead) and list number. The contract term is within 30 days of receiving the purchase order. The contractor must provide and supply a 1-year warranty on material and labor. The contract will be awarded to the most responsible and responsive vendor that provides the lowest cost and meets the requirements. Upon request, the vendor must demonstrate 2 years of relevant experience on similar projects. The awarded vendor must adhere to the time frames, provide certificates of Liability Insurance and Worker's Compensation Insurance, completed Lobbying Law Forms, and a signed Appendix A. All staff working at the project site must sign an attendance sheet. In case of a tie, the contract will be awarded to the vendor whose bid was received earliest. All contracted employees must receive prevailing wage and supplements per NYS Labor Law, and certified payroll records must be submitted with the invoice. Subcontracting is not allowed. HVDDSO will purchase the item #4 Stone and asphalt. The vendor is responsible for trucking, installation, and turning in all scale house tickets. Quotes must be received by the deadline with a detailed cost breakdown using the provided quote proposal form; other forms will not be accepted. Quotes may be emailed, hand delivered, or mailed. Failure to attend the mandatory site visit or submit required documents and forms will result in disqualification. SCOPE OF WORK includes, but is not necessarily limited to the following: 1. The vendor is paving approximately 5,000 sq. feet. 2. The vendor is responsible to verify all measurements. 3. The vendor will grind down top layer of existing driveway. 4. The vendor will be required to excavate, and grade/ slope the entire area of the driveway for proper drainage as needed. Add and compact sub-base as needed. 5. The vendor will supply and apply a layer of tack coat on grinded driveway. 6. The vendor will pave a layer of HMA finish/ surface coarse at minimum thickness of two (2) inches. 7. The vendor is responsible for smooth transition/ grading of butt joints to roadway, walkway, and connecting driveway. 8. Vendor will inspect and stabilized each end of the driveway (beginning of and ending of). Additional details will be presented to vendor on site visit. 9. HVDDSO will provide the item# 4 stone and asphalt for this project. The bidding vendor will provide HVDDSO with material list as part of their bid proposal. 10. The vendor shall provide all required trucking of materials (i.e. Item#4, asphalt) delivered to site, install all materials as required for this project and follow manufacturer's instruction for placement ofmaterials and industry standards. 11. The Vendor shall submit all pick up/delivery tickets to HVDDSO Work Control representative. 12. The vendor will supply all pick up/ delivery tickets included with invoice at the completion of project. Failure to do so will delay in payment.
