There is a mandatory site visit required to participate. When submitting your quote, include a cost breakdown for materials, labor, and overhead, and use the provided proposal form. The contract term is within 30 days of receiving the purchase order. The contractor must provide and supply a 1-year warranty on material and labor. Subcontracting is not allowed. The vendor must demonstrate 2 years of relevant experience on similar projects upon request. The awarded vendor is required to submit certificates of Liability Insurance, Worker's Compensation Insurance, completed Lobbying Law Forms, and signed Appendix A. All work must adhere to the prevailing wage schedule, and certified payroll records must be submitted at invoicing. The vendor must provide all required trucking of materials (Item #4 Stone and asphalt) from the chosen blacktop plant to the project site and install all materials per manufacturer’s instructions and industry standards. The awarded vendor is responsible for submitting all scale house tickets clearly marked with the purchase order number and project name. All quotes, including a detailed cost breakdown, must be received by the deadline listed. Quotes may be hand delivered, mailed, or preferably emailed. Only the provided quote proposal form is accepted. OPWDD/Hudson Valley DDSO's interpretation of specifications is final, and they reserve the right to reject any and all quotes. In the event of a tie, the contract is awarded to the earliest received bid. All project site staff must sign attendance sheets for record purposes.
