Proposal A includes milling and overlay of Penn Avenue North, with approximately 14,580 SY milling of variable depth, about 1,950 tons of hot mix asphalt surface course, traffic striping, and all other necessary or incidental work. Proposal B includes milling, overlay, and reconstruction of portions of Steuben Avenue and Steuben Court, with approximately 3,800 SY milling of variable depth, about 510 tons of hot mix asphalt surface course, traffic striping, and all other necessary or incidental work. Proposal C includes milling, overlay, and reconstruction of Stoney Point Drive, with approximately 3,040 SY milling of variable depth, about 410 tons of hot mix asphalt surface course, traffic striping, and all other necessary or incidental work. Each proposal must be submitted using the forms in the contract documents and enclosed in a sealed envelope bearing the name of the bidder, address, and project name. Materials to be included in the envelope are specified under 'Instructions to Bidders.' Bid proposals must be complete and in ink. Incomplete bids or those not submitted in a sealed, specially addressed envelope may be considered informal. Each bid must be accompanied by a proposal guarantee in the form of a Bid Bond or Certified Check equal to ten percent (10%) of the full amount of each contract bid, not exceeding $20,000, payable to Lacey Township and without condition or qualification. All bidders are required to comply with Chapter 127, P.L. 1975, c. 127, including submitting an Affirmative Action Program of equal opportunity. Bidders must also comply with N.J.S.A. 10:5-31 et. Seq., N.J.A.C. 17:27, the Prevailing Wage Act (N.J.S.A. 34:11-56.25 et seq.), and N.J.S.A. 34:1B-5.1, Chapter 150, Laws of 1963. Corporations or partnerships must submit a statement of ownership for those with a 10 percent or greater interest in the firm.
