Proposers interested in the work must submit one original hard copy and one electronic copy (non-returnable USB flash drive) of their complete bid in accordance with the requirements. All bids and attachments must be bound and delivered sealed to the designated address by the specified deadline. The front lower left corner of each sealed envelope/package must include the project name, ITB number, attention line, and due date. The bidder's name and address must appear on each sealed package, and any multiple packages must be numbered sequentially. Bids should be constructed as outlined, and all required forms can be provided in Word format upon request. The Proposal Transmittal Form on the firm's letterhead must be signed by someone with legal authority to bind the bidder and include supporting documentation if required. The corporate or mailing address must match the information listed on the Florida Department of State Division of Corporations, and proof must be attached. All addenda and tax identification number must be provided. Bid prices must be written for each item, signed in ink, and any alterations must be initialed. If there are mathematical errors, the unit price shall prevail. A bid bond for 5% of the bid amount is required, underwritten by a surety described in the draft agreement.
