Interior and exterior vacant unit rehabilitation at AMP 1, 2206 Catalina Lane, to be completed for the Springfield Housing Authority. The work must be proposed as a firm fixed price for the base bid, with an alternate price for exterior sheathing replacement per square foot. Bidders must submit their bids in a hard-copy format, following the specific tab sequence and index dividers required: SHA Form of Bid, bidder's proposal, HUD 5369A, HUD 2530, Non-collusive Affidavit, Statement of Bidders Qualifications, Contract Compliance Attachment, SHA Hold Harmless Agreement, SHA Special Contract Requirements, SHA Affidavit for Davis Bacon Wage Exclusion, SHA Section 3 Clause, Section 3 Certification forms, Section 3 Workforce Form, Section 3 Workforce Projection Form, and List of Subcontractors (if applicable). All requirements and instructions provided must be carefully reviewed and followed by each bidding firm. Pricing is required for all specified items; no 'no bid' or 'no charge' submissions are accepted for these items.
