Location: FAU 6703 (Country Club Drive). Project Length: 245 FT (0.047 MI). Proposed Improvement: Remove existing steel beam bridge and replace with a single-span PPC deck-beam bridge on concrete bent abutments, hot-mix asphalt approach pavement, and other collateral work. Plans and proposal forms are available at the Tazewell County Engineer office. If required, the two apparent low bidders must file an "Affidavit of Availability" (Form BC 57) in triplicate within 24 hours after the letting, listing all uncompleted contracts and pending low bids. The Awarding Authority may waive technicalities and reject proposals as provided in the BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals. The following BLR Forms must be returned by the bidder: Local Public Agency Formal Contract Proposal (BLR 12200), Schedule of Prices (BLR 12201), Proposal Bid Bond (BLR 12230, if applicable), Apprenticeship or Training Program Certification (BLR 12325 - not for federally funded projects), Affidavit of Illinois Business Office (BLR 12326 - not for federally funded projects). Bid schedule quantities are approximate for comparison only. Contractors will be paid for actual quantities performed and accepted. Bid submission is assurance the bidder has examined the plans and understands all requirements. The bidder is responsible for errors resulting from failure to conduct a detailed examination. Proposals must be filed prior to the specified time and place. Late proposals will be returned unopened. Withdrawal of proposals is allowed if requested in writing or in person before the opening time.
