The Ottawa County Engineer Office is requesting price quotes for the removal and disposal of trees at various locations along county roads. Enclosed is a list of the locations and photographs of the trees at each. All logs, brush and stump material are to be removed from the site. Work is permitted Monday through Friday only. Ottawa County is requesting the work be completed by April 30, 2019. Alternate dates for completions will be considered and should be included on the proposal form. The contractor will be required to provide the county with their work schedule so the county may visit the job site for inspection purposes. Proper signage, flaggers and safety equipment will be required during construction. If your company is interested in furnishing prices for consideration we ask that you provide the following: 1. A complete and signed proposal form 2. Affidavit of no outstanding property taxes and no findings for recovery 3. Non-discrimination statement 4. Additional information that may assist Ottawa County with the selection process (references, experience information, etc.) The entity awarded the contract for this work will be required to provide the following: 1. Hold Harmless Agreement 2. Certificate of insurance listing Ottawa County as additional insured (limitations of required coverage are outlined on the enclosed Insurance Guidelines form) 3. List of subcontractors to be employed by the prime to work on this project (if any) 4. Enter into a written contract 5.
