Public works division is seeking qualified firms to provide professional architectural and/or engineering services for the information and communication services division (icsd) kukuiolono radio facility, kauai - demolition of former hawaii air national guard (hiang) facility project. This project involves the design and construction support services to demolish a telecommunication facility located within the Kukuiolono Park and Golf Course, and removal of all related improvements. Work includes testing for hazardous materials, any site cleanup and remediation work as necessary, and conducting a Phase ll Environmental Site Assessment. Facility description: 13,043 SF area 100' steel lattice tower with guy wires (3 anchors). 8' x 12' fiberglass shelter Above ground propane tank The estimated construction cost is between $500,000 and $1,000,000. Provide design and engineering services including but not limited to full plans, specifications, detailed cost estimate, calculations, basis of design, construction support and other services required for a complete project. 1. Insurance: Firms are not required to submit insurance documentation at this time.However, firms are advised that the listed coverages that follow are required to be awarded a contract after selection for a project: a. General liability insurance for limit of not less than $1,000,000 per occurrence and $2,000,000 aggregate. The STATE shall be named as additional insured. b. Automobile liability insurance for limit of not less than $1,000,000 per accident. The STATE shall be named as additional insured. c. Professional liability insurance for limit of not less than $1,000,000 per claim and $2,000,000 annual aggregate. d. Workers Compensation and Employer's Liability (where required by HRS 386). Minimum limit of liability for workers compensation is the HRS 386 statutory limit.
