Amendment P0004 The purpose of this amendment is to officially and effectively make the following changes/updates listed below: 1. The proposal due date has been extended to close on April 5, 2024 at 10:00 am MDT. All proposals shall be sent to Contract Specialist Angel Waalkes at angela.waalkes@va.gov. All other terms and conditions remain the same. The contractor shall furnish all labor, equipment, materials, and supervision to complete the Urgent Care Renovation Project located at 2121 N. Ave, Grand Jct., CO 81501. Project magnitude: between $5,000,000 and $10,000,000 The NAICS code is 236220 - Small Business size standard is $45 M. In accordance with P.L. 116-283, this acquisition is 100% set aside for Service-Disabled Veteran Owned Small Business concerns. See VAAR 852.219-73, VA Notice of Total Set-Aside for Verified Service-Disabled Veteran-Owned Small Business for information in regard to requirements in order to be considered. Offerors must be both VISIBLE and VERIFIED by the Small Business Administration (SBA) prior to proposal receipt. Failure to be both VERIFIED, and VISIBLE by the SBA prior to contract award will result in the offerors proposal being deemed non-compliant. All offerors are urged to contact the SBA and submit the aforementioned required documents to obtain SBA (VetCert) verification of their SDVOSB status if they have not already done so. All questions in relation to this solicitation must be submitted in writing via e-mail to Angela.Waalkes@va.gov and received no later than January 26th, 2024, at 12:00 PM (NOON)Mountain Time (MT). The construction performance period is 421 calendar days from receipt of notice to proceed. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
