East Hills Reservoir and Booster Pump Station First-tier Subcontractor Disclosure Forms Will Be Received at the Above-mentioned Location and Date, Until 4:00 Pm, Local Time. The Work Contemplated Consists of Furnishing All Labor, Materials and Equipment Necessary for the Construction of a 2.0 Million Gallon Ground-level Welded Steel Reservoir With Associated Site Piping, Stormwater Detention Facilities, and Related Site Work; a 1,200 Gallon Per Minute Firm Capacity Potable Water Pump Station With Associated Electrical, Instrumentation, Controls and Telemetry Systems; Approximately 2,700 Lineal Feet (Lf) of 12-inch Diameter Transmission Main, 550 Lf of 18-inch Diameter Transmission Main, and 1,000 Lf of 16-inch Diameter Horizontal Directional Drilled Transmission Main With Pressure Reducing Valve Vaults and Related Roadway Restoration. The Work Also Includes Connections Into Existing Water Distribution Systems and Demolition of Existing Facilities. A Bidders Failure to Attend the Pre-bid Meeting Shall Cause Any Bid Submitted by That Bidder to Be Deemed Non-responsive, and the Bid Will Be Returned Unopened No Proposal Will Be Considered by the City of Seaside Unless the Bid Contains a Statement by the Bidder That the Bidder Will Comply With the Provisions of Ors 279c.800 to 279c.870 and 40 U.s.c. 276(a) Pertaining to Prevailing Wages, as Applicable. Contractor Licensing Under Ors 468a.720 for Working With Asbestos-containing Materials Is Not a Requirement of This Project. Each Bid Must Contain a Statement as to Whether or Not the Bidder Is a Resident Bidder as Defined in Ors 279a.120. Each Bid Must Also Contain a Statement as to Whether the Bidder Is Registered With the Oregon Construction Contractors Board. No Bid Will Be Received or Considered Unless the Bidder Is Registered With the Construction Contractors Board, as Required by Ors 701.035 Et Seq. No Bid Will Be Considered Unless the First-tier Subcontractor Disclosure Form Is Submitted by the Specified Deadline. Faxed Copies Will Not Be Accepted. Contractors Must Be Qualified in Accordance With the Applicable Parts of Ors 279 in Order to Enter Into a Contract With the City of Seaside for Public Work in Oregon. To Document Qualifications, Bidders Shall Submit a Completely Executed Contractors Prequalification Application When Submitting Their Bid Proposal. Prequalification Application Forms May Be Obtained at the State of Oregon Department of Administrative Services, Purchasing Section, 1225 Ferry Street S.e., Salem, Oregon 97301-4285the City of Seaside Will Investigate and Determine the Qualifications of the Bidders as Part of Its Evaluation of the Bids. Bid Recommendation Will Go to the City Council for Final Approval. Special Minimum Experience Qualifications Apply to Portions of This Project. Pre-bid Approval Reservoir Contractors, Tank Painting Contractors, and Horizontal Directional Drilling Contractors Is a Requirement for This Project. Only Reservoir Contractors, Tank Painting Contractors, and Horizontal Directional Drilling Contractors Who Have Submitted the Required Statements of Qualification and Received Qualification by the Owner's Representative Prior to Bidding May Be Named in the Proposal. Statements of Qualification Complying in Form and Content to the Requirements of the Contract Documents Shall Be Presented to the Owners Representative No Later Than June 12, 2019. The Minimum Qualifications and Submittal Requirements for the Reservoir Contractors, Tank Painting Contractors, and Horizontal Directional Drilling Contractors Are Specified in Section 00 21 13, Instructions to Bidders, of the Contract Documents. Reservoir Contractors, Tank Painting Contractors, and Horizontal Directional Drilling Contractors Previously Qualified by the Owners Representative Are Provided in Section 00 21 13, Instructions to Bidders. Additional Reservoir Contractors, Tank Painting Contractors, and Horizontal Directional Drilling Contractors Qualified by the Owner's Representative Will Be Named by Addendum No Later Than June 14, 2019. Proposals Submitted Naming a Reservoir Contractor, Tank Painting Contractor, and Horizontal Directional Drilling Contractor That Has Not Been Qualified by the Owners Representative Prior to Bidding Shall Be Considered Non-responsive and Shall Be Rejected. All Statements of Qualification Shall Be Sent Directly to Murraysmith, Inc., Attn: Adam Blair, P.e., 888 S.w. 5th Ave., Suite 1170, Portland, or 97204, Phone (503) 225-9010. Proposals Must Be Submitted on the Prescribed Forms and Must Be Accompanied by Certified Check, Cashiers Check, or Bid Bond Executed in Favor of the City of Seaside in an Amount Not Less Than Five Percent (5%) of the Amount Bid. The Successful Bidder Will Be Required to Furnish to the City of Seaside a Performance Bond and a Payment Bond Each Equal to One Hundred Percent (100%) of the Amount of the Bid. The Successful Bidder and All Subcontractors Will Be Required to Furnish to the Oregon Construction Contractors Board a Public Works Bond in the Amount of $30,000, Unless Exempt Under Ors 279c. The City of Seaside Reserves the Right to Reject Any and All Bids, to Waive Any Irregularities, and to Accept the Bid Deemed in the Best Interests of the City of Seaside. The City of Seaside May Reject Any Bid Not in Compliance With All Prescribed Public Bidding Procedures and Requirements, and May Reject for Good Cause Any or All Bids Upon a Finding of the Agency It Is in the Public Interest to Do So
