Site work and new construction of a medical facility in Pontiac, Illinois. Completed plans call for the construction of a 17,644-square-foot medical facility; and for site work for a medical facility.

This project consists of construction of a new Health and Education Building, 17,644 building gross square feet, single story, slab on grade as well as associated site development. This work shall include all labor, supervision, materials, transportation and services necessary and required to perform the Health and Education Building project as set forth in the Contract Documents. All requests for information (RFI) shall be submitted to Farnsworth Group in writing. The project team will maintain a record of RFI's. Written response to RFI's will be provided by Farnsworth Group and shared with all plan holders. Bid security shall be submitted with each bid in the amount of five (5) percent of the bid amount. No bids may be withdrawn for a period of 90 days after opening of bids. Owner reserves the right to reject any and all bids and to waive informalities and irregularities. Bidders must be properly licensed under the laws governing their respective trades and be able to obtain insurance and bonds required for the Work. A Performance Bond, separate Labor and Material Payment Bond, and Insurance in a form acceptable to Owner will be required of the successful Bidder. The Owner requires all contractors and vendors doing business with the Owner not to discriminate against anyone on the basis of race, age, color, religion, gender, sexual orientation, ancestry, non job-related handicaps or national origin. Livingston County Board reserves the right to accept or reject any and all bids, and to waive any and all informalities in the bidding. After receipt of bids and completion of the review process, Livingston County Board may award a contract to the bidder that, in its opinion, will provide a combination of the best services and reasonable cost. Bidders are required to submit certifications of compliance with Non-Collusion, Eligibility to Enter into Public Contracts, Sexual Harassment Policy, Drug Free Workplace, Prevailing Wages and Bidder's Qualifications with their bid. Bids will be held good for a period of ninety (90) subsequent to the opening of bids.
