This Project Includes Evaluation of All Streetlights Locatied in the Panama City Beach Cra District That Are Currently Damaged or Not Operating and a Detailed Report and Estimate to Repair the Lights. The Contractor Shall Provide All Materials, Equipment and Labor to Complete the Project. The Bid Must Conform to Section 287.133(3) Florida Statutes, With Respect to Public Entity Crimes. the City of Panama City Beach (City) Reserves the Right to Reject Any and All Bids. All Bids Shall Be Firm (Including All Labor and Material Prices) for a Period of 30 Days After Opening. All Bidders Shall Be Florida State Certified and Must Provide Copies of Certificate of Insurance and Workman's Compensation. The City Shall Award the Contract to the Lowest Responsive and Responsible Bidder; Provided, However, the City Reserves the Right to Award the Contract to a Bidder Who is Not the Lowest Responsive and Responsible Bidder if the City Determines in Its Reasonable Discretion That Another Bid Offers the City a Better Value Based Upon the Reliability, Quality of Service, or Product of Such Other Bidder. Scope of work 1. Contractor shall perform assessment of all damaged light poles, fixtures, fixture heads, arms, bases and control panels. Said assessment to be signed & sealed by licensed electrician. 2. Repair estimates shall be provided for each light pole, identifying labor, material, and equipment costs for each specific pole. 3. GPS location to 5 decimal places is to be recorded for each pole. 4. Photographs are to be taken of ALL damaged components on each pole. 5. The contractor's reports will be packaged in the form of a site inspection report (SIR), GM Component, and Photo Pages. Templates attached herewith. All the above information is to be recorded in a 3-ring binder tabbed by street location. A CD of same is to be submitted. The light poles to be assessed are listed on the attached spread sheet and are generally located as follows: A. Front Beach Road, including the Hutchison Blvd intersection, from North Thomas Drive to South Thomas Drive. B. South Thomas Drive C. Richard Jackson Blvd from Panama City Beach Parkway to Front Beach Road D. Churchwell Drive including the public parking lot E. The north half of Powell Adams Road
